Seems to me as though maybe we just need a competency test for builders and operators.  :doh:

Hi,

The reason we do an overspeed test is to make sure all the controlls for not allowing it overspeed are working. :)

In this case they didn't work :doh:

This is what happens when the people involved are incompetent. :zap:

When they do not work or are just not part of the system you can get them big globs of oil floating upon the shore. Better to find the fault under test conditions then during production.
